測(cè)量血壓、血糖、體重、心率等生理指標(biāo)。

詢(xún)問(wèn)患者近期癥狀變化,如頭痛、頭暈、心慌、乏力、多飲、多食、多尿等。

了解患者是否按醫(yī)囑規(guī)律服藥、注射胰島素,以及是否存在藥物不良反應(yīng)。

對(duì)于出現(xiàn)危急情況(如收縮壓≥180mmHg和/或舒張壓≥110mmHg,意識(shí)改變、劇烈頭痛或頭暈、惡心、嘔吐、視力模糊、眼痛、心悸、胸悶、喘憋不能平臥等)的患者,須在處理后緊急轉(zhuǎn)診,社區(qū)衛(wèi)生服務(wù)中心(站)應(yīng)在2周內(nèi)主動(dòng)隨訪(fǎng)轉(zhuǎn)診情況。
For patients who experience critical situations (such as systolic blood pressure ≥ 180mmHg and/or diastolic blood pressure ≥ 110mmHg, changes in consciousness, severe headaches or dizziness, nausea, vomiting, blurred vision, eye pain, palpitations, chest tightness, wheezing and inability to lie flat, etc.), emergency referral must be made after treatment, and community health service centers (stations) should actively follow up on the referral situation within 2 weeks.
